The single hardest filler to do well.
The tear trough is the most technique-sensitive filler placement on the face. Done well, it removes the tired look that no concealer can fix. Done poorly, it produces the lumpy, blue-tinged 'Tyndall effect' that's now a meme on Reddit's r/PlasticSurgery.
We do tear troughs by cannula, not needle, in the vast majority of cases. We use lower-G-prime HA placed deep on bone — never superficially in thin undereye skin. And we will turn patients away who aren't anatomical candidates.

Undereye filler is HA placed in the deep fat compartment under the orbital rim, supporting the skin where shadow forms. Suitable products are low-G-prime, low-hydrophilic gels — Restylane-L and RHA 2 are our usual choices. We do NOT use Voluma here.
Not every dark circle is a candidate for filler. Pigmentation-driven darkness, allergic shiners, and skin-quality issues need different approaches. We diagnose first, treat second.

Consultation
Photograph with overhead light to identify shadow source. Many patients are not candidates — we will tell you.

Treatment
30 minutes. Cannula entry point at lateral cheek, conservative product placement. We typically use 0.5–1.0 mL total across both sides.

Recovery
Bruising at cannula entry possible. Avoid blood thinners and alcohol pre-treatment.

Results
Immediate, settles at 2 weeks. Patients sometimes need a small touch-up at the 2-week mark.
Who's a candidate
Undereye Filler is right for you if:
- Patients with true tear-trough hollow (volume loss creating shadow)
- Those whose darkness improves when they pull their lower lid up
Not for you if
We'll turn you down if:
- Patients with thin, pigmented undereye skin without volume loss
- Festoons or eye-bags from fat herniation (surgical problem)
- Patients with prior lower-lid surgery — case-by-case
