Plain-English summary

The web forms on this site (booking, contact) collect logistical information only — your name, email, phone, treatment of interest, and a logistics-only notes field. We do not collect any health information through this site, and we ask that you do not submit any. Health information is collected only in person, at the consultation, where it can be handled in compliance with privacy law.

What we collect

From the website: your form submissions (name, email, phone, treatment interest, source, optional logistics notes); standard analytics (anonymous device/browser data, page views, referrer); and cookies necessary to operate the site.

From your visit to the clinic: medical history you provide on intake, photographic documentation of your treatment areas (with separate consent), and treatment records. This information is held in our HIPAA-compliant electronic medical records system.

HIPAA notice

Lumère Medical Aesthetics is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information (PHI) is held only in HIPAA-compliant systems with executed Business Associate Agreements. Do not send PHI through this website's forms or via standard email. If you need to communicate sensitive information, ask our concierge for our secure patient portal link, which provides a HIPAA-compliant message channel.

You have the right to receive a copy of our full Notice of Privacy Practices at the clinic. We make it available at intake and on request.

Sharing & vendors

We share information with vendors who provide services we need to run the practice — payment processors, scheduling software, our HIPAA-compliant EMR, and our laboratory partners. Each vendor is bound by either a Business Associate Agreement (for PHI) or appropriate privacy contracts.

We do not sell your information. We do not share your information with marketing partners outside the bounds of services you have specifically opted into.

Your rights

Under HIPAA and Florida law, you have the right to: access your own records; request corrections; receive an accounting of disclosures; restrict certain uses; receive confidential communications; and file a complaint without fear of retaliation.
